Please be patient as they receive our enrollment information from Preferred and get all of our information loaded into their system. You will receive an ID card over the next couple of weeks. Until then, please have your providers send all claims to the following address:

If your provider needs to verify coverage, or has any questions on the plan, they may call Yohan directly at (203) 647-0626
Once you receive your new ID Card in the mail, please give it to your provider the next time you need any services. This card will instruct the provider how to submit electronically and the proper mailing address.
